The firehouse myth: “Coffee doesn’t count”
Coffee and shift work are almost inseparable. It is there for early starts, late reports, interrupted nights, and the reset after a difficult call. Yet coffee is often treated as if every cup must be “paid back” with extra water because caffeine supposedly removes more fluid than the drink provides.
That belief starts with a real effect: caffeine can increase urine production. But a temporary diuretic response is not the same as becoming dehydrated. The practical question is whether drinking coffee produces a meaningful net loss of body water under normal conditions.
What the research actually shows
In a counterbalanced crossover study, 50 habitual male coffee drinkers consumed either four 200 mL servings of coffee providing 4 mg of caffeine per kilogram of body mass per day or the same volume of water. Researchers found no significant differences between the coffee and water conditions across total body water and multiple blood and urine markers of hydration. The authors concluded that moderate coffee intake provided similar hydrating qualities to water in these caffeine-habituated participants.
That result does not mean caffeine has no diuretic effect. A separate trial comparing lower- and higher-caffeine coffee found that the higher dose increased short-term urine output more than the lower dose. Dose matters, and people who rarely consume caffeine may notice a stronger response than regular coffee drinkers.

When should you pay closer attention?

1. When the dose climbs
A normal serving and a very high caffeine intake are not interchangeable. More caffeine does not automatically produce more benefit, and higher doses are more likely to cause side effects. Coffee caffeine content also varies by bean, roast, serving size, and brewing method, so counting cups is only an estimate.
2. During heat, heavy sweating, or long calls
Coffee still provides fluid, but it does not replace the water and sodium lost through substantial sweating. During hot-weather work, strenuous training, or extended incidents, use coffee for what it does well and maintain a separate plan for fluid and electrolyte replacement.
3. When caffeine affects sleep
For shift workers, the bigger drawback may be sleep rather than hydration. A 2023 systematic review and meta-analysis found that caffeine reduced total sleep time and sleep efficiency on average. Timing and sensitivity vary, but using caffeine late in a shift can make the next recovery window harder—even if it helps in the moment.
4. When your body tells you the dose is too high
Jitters, anxiety, stomach discomfort, a racing heartbeat, and disrupted sleep are reasons to reduce or retime caffeine. Individual responses differ, and coffee should support the shift rather than create another problem to manage.
